The effects of community policies to reduce youth access to tobacco.

OBJECTIVES: This study tested the hypothesis that adoption andimplementation of local policies regarding youth access to tobaccocan affect adolescent smoking. METHODS: A randomized communitytrial was conducted in 14 Minnesota communities. Seven interventioncommunities participated in a 32-month community-organizingeffort to mobilize citizens and activate the community. Thegoal was to change ordinances, merchant policies and practices,and enforcement practices to reduce youth access to tobacco.Outcome measures were derived from surveys of students beforeand after the intervention and from tobacco purchase attemptsin all retail outlets in the communities. Data analyses usedmixed-model regression to account for the clustering withincommunities and to adjust for covariates. RESULTS: Each interventioncommunity passed a comprehensive youth access ordinance. Interventioncommunities showed less pronounced increases in adolescent dailysmoking relative to control communities. Tobacco purchase successdeclined somewhat more in intervention than control communitiesduring the study period, but this difference was not statisticallysignificant. CONCLUSIONS: This study provides compelling evidencethat policies designed to reduce youth access to tobacco canhave a significant effect on adolescent smoking rates.
