Pitfalls of and Controversies in Cluster Randomization Trials

It is now well known that standard statistical procedures becomeinvalidated when applied to cluster randomized trials in whichthe unit of inference is the individual. A resulting consequenceis that researchers conducting such trials are faced with amultitude of design choices, including selection of the primaryunit of inference, the degree to which clusters should be matchedor stratified by prognostic factors at baseline, and decisionsrelated to cluster subsampling. Moreover, application of ethicalprinciples developed for individually randomized trials mayalso require modification.
We discuss several topics related to these issues, with emphasison the choices that must be made in the planning stages of atrial and on some potential pitfalls to be avoided.
